The trouble, imo, is the sub-editors (or whoever is tasked with it) who add titles which fail to reflect the content of the story, and often downright contradict it.
The trouble is that publications base how and what they report on analytics of individual articles in a way they couldn't with newspapers. Their earnings are also dependent on it.
And people read rubbish like this from the Mail, Express, Sun etc. far more than they would read a story that took a week to report on.
Loughton, I haven't heard of it either but if it's with the IPCC then many tabloids aren't signed up.
There is no binding code of ethics, they exist in different forms but are rarely followed. For example, I'm signed up to a code but only because I am a member of the National Union of Journalists but they have very few members.

I do tend to think the owners (well one) get a bit carried away and some of the fans take every word literally.
